All new set up requests must be submitted using the new version of the following documents: Be sure to review the New Wright Express Application Requirements and complete the W9 section of the WEX form. A legible pre-printed voided check or Bank letter is required. Starter checks and deposit slips are not accepted. If the business is located in California but corporate offices or owner residence is outside California a CA FTB 590 Form is required.
